During the underwriting process, it may be necessary for the insurance company to review your medical records. Some insurance companies provide a valuable service by obtaining and paying* for the medical records on your behalf.

YES =	The insurance company provides a valuable service by obtaining and paying* for any needed medical records.
NO =	You will be responsible for obtaining and paying for any needed medical records.
N/A =	Not applicable; the insurance company does not require medical records.
